Is Ultherapy the same as HIFU?
Ultherapy is a form of HIFU. Its maker describes the technology as micro-focused ultrasound with visualisation (MFU-V), but the physical principle is identical to that of every HIFU device. An ultrasound beam is focused, much as a lens focuses light, into tiny points of heat at a chosen depth, while the skin surface remains intact. Each point is heated above about 60°C, the temperature at which collagen fibres contract. The tissue then repairs each point over the following 2 to 3 months by forming new collagen, which tightens and lifts the overlying skin. At the deepest Ultherapy setting of 4.5 mm, the heat reaches the superficial musculoaponeurotic system (SMAS), the fibrous muscle layer that surgeons tighten in a facelift.
A Singapore clinic that advertises “HIFU” without naming a brand is usually using a Korean device such as Ultraformer MPT, Ultracel Q+, Doublo or Liftera. Ultherapy and these devices differ in imaging, regulatory approval, published evidence and price, rather than in the way they heat tissue.

The real differences between Ultherapy and HIFU
- Imaging Ultherapy displays a live ultrasound image of the skin, fat, muscle layer and bone before each line is delivered. Its US labelling states two purposes for the image, confirming that the transducer is in full contact with the skin and setting the depth so that bone is avoided. Generic HIFU is applied at a fixed depth determined by the cartridge, and accurate placement therefore depends on the operator’s knowledge of the anatomy. Because skin and fat thickness vary between patients and between areas of the face, a fixed depth may deposit the heat in a different layer from the one intended.
- Regulatory status Ultherapy has held US FDA clearance for lifting the eyebrow since 2009, for the under-chin and neck since 2012 and for décolleté lines since 2014. Ultraformer received Korean approval and a European CE mark in 2014, and its MPT version received Korean approval in 2021 and a CE mark in 2025. Neither device is cleared in the United States for treatment of the eyelids.
- Evidence Ultherapy is the most extensively studied focused-ultrasound brand and is the subject of two meta-analyses. Generic devices are supported by smaller, mostly Korean studies, several of which have manufacturer co-authors, a conflict of interest that lowers confidence in the reported results.
- Price A full-face Ultherapy session costs S$1,800 to 3,700 in Singapore, against S$590 to 2,000 for generic HIFU. The difference reflects the branded system and its transducers rather than any demonstrated difference in result.
Ultherapy vs HIFU results in published studies
- EvidenceEvidence for Ultherapy on the face and neck, Grade A. In a 2025 systematic review of 42 studies, 89% of patients were rated as improved by investigators and 84% reported improvement themselves. A 2023 meta-analysis of 13 studies reported a response rate of 77% at 90 days, falling to 69% at 180 days. Most of the pooled studies lacked a control group, so the share of the improvement attributable to treatment alone is uncertain.
- EvidenceEvidence for generic HIFU on the face, Grade B. A 2020 meta-analysis of 17 studies across several devices found a moderate improvement of about 2.7 on a 5-point scale. In a Korean study of 20 patients, blinded clinicians rated the jawline, cheeks and perioral area as the most improved regions at 3 to 6 months. A three-dimensional scan study of 50 patients treated with Ultraformer MPT measured an immediate lift of 2.9 mm at the jowl and 2.25 mm at the crow’s feet. This lift was recorded before any collagen remodelling could occur, which indicates that the immediate effect arises from heat contraction of existing collagen.
- Ultherapy vs HIFU head to head, Grade D No published study has compared Ultherapy with Ultraformer, Ultracel, Doublo or any other generic HIFU device. Claims that either device lifts more than the other therefore rest on comparisons between separate studies, which are unreliable because those studies differ in patients, treated areas and outcome measures.
Ultherapy vs HIFU for the eyes
- Brow and upper-lid hooding A rater-blinded study of 35 patients treated with Ultherapy measured a mean brow lift of 1.7 mm at 90 days (Grade B). In 25 Asian patients given a full-face and neck treatment, the brow rose by 0.47 mm at 90 days and had returned to baseline by six months.
- Eyelids In a 2026 prospective study, 34 Korean women received one session of Ultraformer MPT around the eyes with a 2.0 mm cartridge. At 12 weeks, upper-lid length, measured from the brow to the lid margin, had shortened by 0.94 mm, 76% of patients reported improvement and blinded clinicians recognised improvement in 59%. One author is employed by the manufacturer (Grade C).
- Under-eye The only Ultherapy lower-lid study, published in 2014, treated seven patients directly on the lower lid and measured a change of about 0.5 mm (Grade C). Ultherapy’s current instructions for use advise against treatment on the eyelids or within the orbital rim.

Ultherapy is priced by the number of lines and generic HIFU by the number of shots, each line or shot being one pass of the transducer that deposits a row of heat points. Two quotes are therefore comparable only when each states the device and the count. Several clinics advertise “HIFU” without naming the machine, and some listed prices exclude goods and services tax (GST).
Ultherapy PRIME vs HIFU
Ultherapy PRIME is the newer Ultherapy console, cleared by the FDA in February 2024 and updated in 2025. It applies the same micro-focused ultrasound principle with built-in imaging, and its advantages over generic HIFU are therefore the same as those of Ultherapy in general, namely imaging and regulatory clearance. No study has compared PRIME with the original Ultherapy console or with any generic HIFU device.
Ultherapy and HIFU safety around the eyes
- Eye injury Published case reports describe damage to the cornea, iris or lens, or a rise in eye pressure, after focused ultrasound was applied on or near the eyelids. Because eyelid skin is very thin, a focal depth chosen for facial tissue can place the heat inside the eye rather than in the lid. One patient developed a cataract within days and required lens surgery, and another was left with a permanent corneal scar. These injuries are rare, but they account for the standard precautions of keeping treatment outside the orbital rim and using protective eye shields.
- Nerves Temporary numbness or muscle weakness caused by irritation of a nerve has been reported after Ultherapy.
- Common effects Redness, swelling, tenderness and occasional welts or bruising follow either treatment and usually resolve within days.
Ultherapy vs HIFU, which to choose by concern and budget
- Brow lift or deep jawline and neck tightening, with budget for the brand. Ultherapy is the better-supported option, because its imaging confirms the treatment depth and its evidence base is larger.
- Mild laxity at a lower cost Generic HIFU applies the same heating principle at a lower price, although its supporting studies are smaller.
- Loose eyelid skin Thermage Eyes has stronger evidence than either device and, unlike focused ultrasound, is cleared for use on the eyelids themselves. The two are compared in Thermage vs Ultherapy.
- Heavy jowls or skin resting on the lashes Surgery is usually required, because focused ultrasound tightens tissue but cannot remove excess skin.
Frequently asked questions about Ultherapy and HIFU
Is Ultherapy better than HIFU?
Ultherapy has not been shown to give a better result than generic HIFU, because no study has compared them directly. Its advantages lie in live imaging, US clearance and a larger body of published studies.
Why is Ultherapy more expensive than HIFU?
Ultherapy is a single branded system with built-in imaging and proprietary transducers, and clinics price it by the number of lines. Generic HIFU machines cost less to run, so a full face costs about S$590 to 2,000, against S$1,800 to 3,700 for Ultherapy.
Is HIFU safe around the eyes?
HIFU applied near the eyes has caused rare but serious eye injuries, including cataract and corneal scarring, in published case reports. Treatment is therefore kept outside the orbital rim, with protective eye shields in place, and should be performed by a trained doctor.
Is Ultherapy PRIME better than HIFU?
Ultherapy PRIME has not been shown to outperform HIFU, because no study has compared it with generic HIFU devices. It is the newer Ultherapy console and shares the same imaging advantage.
How often should HIFU be done?
Most protocols use a single session, repeated after about a year because skin laxity continues to progress with age. Some Korean studies used a course of three sessions four weeks apart.
