Sunken eyes treatment options in Singapore
A sunken eye is a volume problem, in which the hollow sits where fat, bone or cheek support has gone or was never there. Treatment therefore either adds volume into the hollow or improves the skin over it so the shadow is less visible. The cause determines which, and is worked through at What causes sunken eyes. Dermal fillers for sunken eyes, hyaluronic acid tear trough filler
A soft, low-swelling hyaluronic acid gel is injected deep in the tear trough, on or just above the bone, with a blunt cannula or a fine needle. It lifts the lower lid out of the hollow and removes the shadow. Most people need 0.5 to 1 ml per side, and the treatment takes 20 to 30 minutes with numbing cream. Products designed for the area are thinner and attract less water than cheek fillers, which keeps swelling down.
- Who it suits. A tear trough or hollow with reasonably thick skin and no large eye bag above it. It is the best option for the grade 1 to 3 tear trough described on the causes page, for weight-loss hollows once weight is stable, and for mild over-removal after surgery.
- Evidence Grade A. Several randomised controlled trials and systematic reviews show a measurable improvement in tear trough depth that persists at 12 months in most patients, with high satisfaction and a low rate of serious complications when injected by trained doctors.
- 2026 Singapore price. S$600-1,200 per syringe. Most people need one syringe for both sides, some need two.
- Sessions. One, with a review at two weeks for a touch-up.
- Downtime. 1 to 3 days of swelling, with bruising in a minority lasting 5 to 7 days.
- Longevity. 9 to 18 months. The area moves little, so filler often lasts longer here than elsewhere.
- Reversibility. Yes, with hyaluronidase within hours.
- Risks specific to the under-eye. The Tyndall effect, a blue-grey tint through thin skin when the gel is placed too superficially, which can last as long as the filler and is treated by dissolving it. Persistent puffiness or “malar bags” if too much is used or the product attracts water. Lumps and unevenness. Rarely, vascular occlusion if filler enters an artery, which can cause skin loss or, in very rare cases, blindness. The angular and infraorbital arteries run through this area, so the tear trough is regarded as one of the higher-risk filler sites and cannula technique with small volumes is the norm.
Fat grafting for sunken eyes
Fat is harvested from the abdomen or thigh with a small liposuction cannula, washed or filtered, and injected into the tear trough and upper cheek in fine strands. Some of the fat establishes a blood supply and becomes permanent. Nano-fat and micro-fat variants use finer preparation for the thin under-eye skin.
- Who it suits. Marked hollowing, deep-set eyes, hollowing after eye bag surgery, patients who want a longer-lasting result than filler, and anyone having lower blepharoplasty at the same time. It needs a surgeon and a procedure room, and is less predictable than filler in the amount that survives.
- Evidence Grade B. Cohort studies show lasting improvement in tear trough depth with 50 to 70 per cent of injected fat surviving at one year. No randomised trial compares it with hyaluronic acid.
- 2026 Singapore price. From S$3,500, typically S$3,500-8,000 for the eye area alone, more when combined with other areas or surgery.
- Sessions. One, with a second session in 20 to 30 per cent to top up.
- Downtime. 1 to 2 weeks of swelling and bruising at the eye and the donor site.
- Longevity. Years, for the fat that survives.
- Reversibility. No.
- Risks. Lumps and irregularity that are harder to correct than filler, over-correction that persists, asymmetry from uneven survival, donor-site bruising, and the same rare vascular risk as filler.
Polynucleotide and skin booster injections for sunken eyes
Polynucleotide injections (Rejuran I, the eye-area version) and hybrid boosters (Juvelook, a poly-D,L-lactic acid and hyaluronic acid mix, and similar products) are injected in small droplets into the dermis of the lower lid. They are marketed as improving skin thickness, fine lines and texture rather than as volume fillers. On a sunken eye they make the thin, crepey skin over the hollow look healthier and slightly fuller, but they do not lift the lid out of the groove.
- Who it suits. A mild hollow with thin skin and fine lines, as an adjunct to filler, or for patients who are not suitable for or do not want filler.
- Evidence Grade C. Small studies and case series report improvement in skin elasticity and fine lines after three sessions, and there is no controlled evidence that they reduce tear trough depth.
- 2026 Singapore price. S$500-1,200 per session.
- Sessions. 2 to 3, two to four weeks apart, then maintenance every 6 to 12 months.
- Downtime. 1 to 3 days of small bumps at the injection points, bruising in some.
- Longevity. 6 to 12 months of skin effect.
- Risks. Bruising, bumps that persist a few days, and, for Juvelook, nodules if placed too shallow.
Collagen-stimulating fillers under the eye
Calcium hydroxylapatite (Radiesse) and poly-L-lactic acid (Sculptra) stimulate the skin to produce collagen and are used for cheeks and temples. They are generally not recommended in the tear trough. The skin here is the thinnest on the face, the product is not dissolvable, and the published rate of visible nodules is higher than anywhere else. Some clinics offer heavily diluted preparations, but this is off-label and the evidence is small case series only.
- Evidence Grade C, with a caution. The available series report nodules and prolonged swelling in a meaningful minority, and there is no antidote.
- 2026 Singapore price. S$900-2,000 per syringe where offered. A hollow that would need a collagen stimulator is usually better served by fat grafting or by filling the cheek above with a stimulator and the trough itself with hyaluronic acid.
Eye creams for sunken eyes
Eye creams with caffeine, peptides, retinol or hyaluronic acid hydrate the surface and, in the case of retinol over months, can thicken the dermis slightly. None adds volume. A cream can make the skin over the hollow look smoother and reflect light better, which reduces the shadow a little for a few hours. Light-reflecting concealer applied only in the groove does more.
- Evidence Grade D for a hollow. There is no published evidence that any topical product reduces tear trough depth. Retinoids have Grade B evidence for fine lines and skin thickness, which is a different claim.

- Risks. Irritation from retinol on thin eyelid skin.

How to fix sunken eyes, choosing a treatment in Singapore
The self-check on What causes sunken eyes confirms whether the hollow is structural, since a hollow that changes with sleep and fluid needs no treatment. An eye bag above the trough changes the plan, because filling a trough beneath a bag enlarges the bag, which then needs the options at Eye bag removal in Singapore. For a plain tear trough, hyaluronic acid filler is the first choice on evidence, reversibility and cost. For a deep hollow, deep-set eyes or a hollow after surgery, fat grafting gives more volume for longer at the price of a surgical recovery.
The questions that matter when choosing an injector are the number of tear troughs treated a month, whether a cannula is used, which product, what is done if a blue tint or a lump appears, and whether the clinic stocks hyaluronidase. The price difference between clinics is smaller than the difference in outcome between an experienced and an occasional injector in this area. Any offer of a permanent or semi-permanent filler in the tear trough should be declined.

Frequently asked questions
How much does tear trough filler cost in Singapore?
Published 2026 clinic prices are S$600-1,200 per syringe, and most people need one syringe for both eyes. A touch-up at two weeks is usually included or charged at a lower rate. Fat grafting starts at around S$3,500.
How long does under-eye filler last?
Nine to eighteen months. The tear trough moves less than the lips or cheeks, so hyaluronic acid often lasts longer here. Repeat treatments tend to need less product.
What is the Tyndall effect and can it be fixed?
A blue-grey tint through the skin when filler is placed too close to the surface. It can be reversed within a day or two by injecting hyaluronidase, which dissolves the gel. It is the main reason tear trough filler is placed deep, on the bone.
Is tear trough filler safe?
In trained hands serious complications are rare, and the gel is reversible. The specific risks are bruising, swelling, a blue tint, lumps, and, very rarely, blockage of a blood vessel. Cannula technique, small volumes and an injector who treats the area regularly reduce all of them.
Does Rejuran fix sunken eyes?
It improves the skin over the hollow, so fine lines and crepiness look better, but it does not fill the groove. For a visible hollow, hyaluronic acid filler or fat grafting is needed, with Rejuran as an optional add-on for skin quality.
