Droopy eyelid treatment by cause, non-surgical or surgery
The three causes of a sagging eyelid determine the treatment group, and the sagging eyelids pillar covers the condition as a whole.
| Cause | Non-surgical options | Surgical options |
|---|---|---|
| Excess skin (dermatochalasis), mild | Needle RF (AGNES + Secret RF), XERF, Thermage Eyes, HIFU | Not needed yet |
| Excess skin, moderate to severe | Will under-treat | Upper blepharoplasty, infrabrow excision |
| Low lid edge (ptosis) | None | Ptosis repair |
| Dropped brow | Botox brow lift, HIFU or RF to the forehead | Brow lift, infrabrow excision for mild cases |
Prices are 2026 Singapore ranges from at least three published clinic price pages per option, and no clinic is named. Surgical prices are surgeon and facility fees combined, while GST, consultation and anaesthetist fees vary.
AGNES and Secret RF for upper eyelids price Singapore
Two needle-radiofrequency devices used together. AGNES delivers heat through an insulated needle to the deeper tissue and small fat pockets of the lid, and Secret RF uses an array of fine microneedles to heat the dermis and contract and thicken the skin. Local anaesthetic cream and injections are used.
- Who it suits. Mild hooding in patients who want a modest, natural tightening with little downtime, and younger patients with early laxity.
- Evidence Grade C. Small case series report measurable tightening of upper-lid skin and patient satisfaction at 3-6 months, with no controlled trials against surgery or against other devices for the upper lid.
- 2026 Singapore price. S$800-2,000 per session for the eye area.
- Sessions. 1-3, four to six weeks apart, then maintenance yearly.
- Downtime. Swelling 2-4 days, needle marks and pinpoint scabs up to a week, bruising in some.
- Risks. Bruising, temporary dents, small burns, pigment change in darker skin.
XERF eyelid tightening price Singapore
XERF is a Korean mono-polar radiofrequency device that heats the deeper dermis and the fibrous septae under it in a single pass without needles. A dedicated small eye tip is used over the upper lid and brow to contract collagen and stimulate new collagen over 2-3 months.
- Who it suits. Mild to moderate skin laxity of the upper lid and a slightly heavy brow, in patients who want no needles and no visible downtime.
- Evidence Grade C. Only small case series and expert consensus have been published for XERF, and none specifically for the eyelid. Mono-polar RF as a class has Grade B evidence for facial skin tightening.
- 2026 Singapore price. S$1,500-2,500 per session for the eye area, from about S$2,000 for the face.
- Sessions. 1, repeated at 9-12 months.
- Downtime. Redness for 1-2 hours, with make-up the same day.
- Risks. Heat discomfort during treatment, rare surface burns, temporary swelling.
Thermage Eyes price Singapore (Thermage FLX eye tip)
Thermage FLX is the original mono-polar radiofrequency device. The 0.25 cm² eye tip delivers around 450 pulses over the upper and lower lids with the eye protected by a plastic shield. Collagen contracts immediately and new collagen forms over 3-6 months.
- Who it suits. Mild to moderate hooding and lid crepiness in patients who want a single-session treatment with a track record.
- Evidence Grade B. Controlled and cohort studies, including photographic measurement of brow and lid position, show modest lifting of the upper lid (about 1-2 mm) and improved skin texture at 6 months in most patients, with results fading over 1-2 years.
- 2026 Singapore price. S$1,500-2,800 for the eye tip alone, S$2,800-4,500 when combined with face.
- Sessions. 1, repeated every 1-2 years.
- Downtime. Redness and mild swelling for a day or two.
- Risks. Pain during treatment, rare dents or burns, temporary numbness.
HIFU for sagging eyelids price Singapore
High-intensity focused ultrasound (Ultherapy, Ultraformer and similar devices) focuses heat at 1.5-2 mm under the skin using a small peri-orbital cartridge to tighten the dermis of the lid and lift the brow slightly.
- Who it suits. Mild laxity and a slightly heavy brow, as a lower-cost alternative to mono-polar RF.
- Evidence Grade C for the eyelid. A few small studies measured a 1-2 mm brow lift and improved lid skin at 90 days, and the Grade B evidence for HIFU concerns the brow and lower face, not the lid itself.
- 2026 Singapore price. S$300-1,200 for the eye and brow area, depending on the device.
- Sessions. 1-2, repeated every 12-18 months.
- Downtime. None to a day of mild swelling.
- Risks. Pain, temporary numbness and bruising. Eye shields are mandatory to protect the globe.
Botox brow lift price Singapore
Small doses of botulinum toxin are injected into the muscles that pull the brow down (the outer orbicularis and the glabellar complex) so the forehead muscle lifts the brow unopposed. The outer brow rises 1-3 mm and the lid skin follows.
- Who it suits. Mild brow ptosis and mild hooding of the outer lid in younger patients, and as a trial before deciding on surgery.
- Evidence Grade B. Controlled studies document a measurable brow elevation of 1-3 mm lasting 3-4 months.
- 2026 Singapore price. S$200-600 per treatment, depending on brand and units.
- Sessions. Every 3-4 months.
- Downtime. None, with pinpoint bruising possible.
- Risks. An over-arched “Spock” brow, and eyelid droop if the toxin spreads to the levator, which wears off in weeks.
Infrabrow excision (sub-brow lift) price Singapore
A strip of skin and sometimes a little muscle is removed from just beneath the eyebrow, with the scar hidden along the lower border of the brow hairs. It removes outer-lid hooding and lifts the lateral brow without changing the eyelid crease. It is popular in Korea, Japan and Singapore for East Asian lids with thick skin and a low crease, where a standard blepharoplasty can look unnatural.
- Who it suits. Patients over 40 with heavy, thick-skinned outer lids, a low brow, and a wish to keep their existing crease. Patients with thin brows or a high brow are poor candidates because the scar is less well hidden.
- Evidence Grade B. Cohort studies with several hundred patients report durable correction of hooding, high satisfaction and well-hidden scars, with no controlled comparison against upper blepharoplasty.
- 2026 Singapore price. S$3,500-6,500 for both sides under local anaesthesia. No Singapore clinic publishes a dedicated price, so it is priced in the same band as an upper blepharoplasty.
- Sessions. 1.
- Downtime. Stitches out at 5-7 days, bruising and swelling 1-2 weeks, scar matures over 6-12 months.
- Risks. A visible scar below the brow, especially in sparse brows, asymmetry, numbness above the scar.
Drooping eyelids surgery, upper blepharoplasty cost Singapore
The excess skin, and where needed a strip of muscle and some fat, is removed through an incision hidden in the eyelid crease. A crease can be created or raised at the same time (the “double eyelid” variant). It is done under local anaesthesia with or without sedation and takes about an hour.
- Who it suits. Moderate to severe dermatochalasis at any age, and anyone whose hooding is reducing their field of vision. It does not correct ptosis, which must be repaired at the same time if present.
- Evidence Grade A. Systematic reviews and many controlled studies show reliable, long-lasting removal of excess skin, improvement in visual field where it was reduced, and high satisfaction, with a complication rate of a few per cent.
- 2026 Singapore price. S$3,000-6,000 for both eyes at private clinics and hospitals. The MOH fee benchmark for the surgeon’s fee alone (both eyes, private sector) is S$3,400-5,900, so hospital-based or complex cases exceed the clinic range.
- Sessions. 1, with results lasting 10 years or more.
- Downtime. Stitches out at 5-7 days, bruising and swelling 1-2 weeks, final result at 3-6 months.
- Risks. Asymmetry, visible scar, over-correction leaving the eye unable to close fully, dry eye, and rarely bleeding behind the eye.
Ptosis surgery cost Singapore
The stretched levator aponeurosis is shortened and re-attached to the tarsal plate (levator advancement), or a strip of the Müller muscle and conjunctiva is removed from the inside of the lid (Müller muscle resection or conjunctival mullerectomy) for mild cases. Excess skin is often removed at the same sitting.
- Who it suits. Any patient whose lid edge sits low on the eye on the self-check, whether or not there is also excess skin. It is the only treatment that raises the lid itself.
- Evidence Grade A. Systematic reviews and controlled studies show predictable lid elevation with success rates above 85-90 per cent, with re-operation in roughly one in ten for under- or over-correction.
- 2026 Singapore price. S$6,000-9,000 for the surgeon’s fee alone (MOH benchmark, bilateral levator resection, private sector) and a median private-hospital bill of about S$17,000 all-in; no published Singapore clinic price sits below S$5,000-6,000. Where visual field loss is documented, part of the cost may be claimable.
- Sessions. 1, with a revision rate of about 10 per cent.
- Downtime. 1-2 weeks of bruising and swelling, lid height settles over 4-8 weeks.
- Risks. Under- or over-correction, asymmetry, dry eye, contour irregularity, and the need for a second adjustment.
Droopy eyelid exercises, creams and eyelid lifters, do they work?
These are the most searched droopy eyelid remedies in Singapore and the least effective. Droopy eyelid exercises (holding the brow down while raising the lid, rapid blinking, resistance against a finger) cannot shorten excess skin or tighten a stretched levator tendon, because the muscle that lifts the lid is already working at full stretch. Evidence Grade D. No published study shows a measurable change in lid height from exercises.
Droopy eyelid creams sold as “eyelid lifters” or firming serums hydrate the thin lid skin and can make it look slightly smoother for a few hours. They do not remove a skin fold or raise the lid margin. Evidence Grade D. The exception is prescription oxymetazoline 0.1% eye drops, approved overseas for mild acquired ptosis. They stimulate Müller’s muscle and lift the lid by about 1 mm for several hours (Grade A for that specific use), a daily, temporary effect, and they are not available over the counter in Singapore.
Adhesive eyelid lifter strips and tapes work the same way as double eyelid tape, folding the skin for the day and removed at night. They suit occasions, not treatment. These products cost S$20-80 and produce no lasting change, and the options above are what change the lid.

How to fix a droopy eyelid, choosing a treatment
- Check the lid edge first. If the upper lid covers more than the top 2 mm of the iris with the brow held still, the diagnosis is ptosis and only surgery will raise it. Radiofrequency or HIFU bought first is wasted on a ptotic lid.
- Lift the brow with a finger. If the hooding goes, a Botox brow lift or brow-directed HIFU or RF is the logical first step, and infrabrow excision or a brow lift the surgical route.
- For a skin fold alone, grade it against the sagging eyelids causes page. Skin that is slightly loose but not resting on the lashes can be tightened by Thermage Eyes, XERF, needle RF or HIFU, with 1-2 mm of improvement expected for 1-2 years. Skin resting on the lashes needs blepharoplasty or infrabrow excision.
- Choose between blepharoplasty and infrabrow excision by skin thickness, brow height and whether the existing crease is to be kept. Thick-skinned outer hooding with a low brow favours infrabrow excision, while thin skin or a wish for a higher crease favours blepharoplasty.
- If vision is affected, a visual field test before surgery supports any insurance or MediSave claim.
- Lower-lid problems are separate, see sagging skin under the eyes and eye bags.
Frequently asked questions
How much does droopy eyelid surgery cost in Singapore?
Upper blepharoplasty costs S$3,000-6,000 for both eyes at private clinics in 2026 and ptosis repair from S$6,000 (surgeon’s fee) to about S$17,000 all-in in a private hospital. Surgery done for documented visual field loss may be partly claimable.
Can Thermage lift sagging eyelids?
Thermage Eyes tightens mild upper-lid skin and lifts the lid by roughly 1-2 mm for 1-2 years, with Grade B evidence. It cannot correct ptosis or remove skin that rests on the lashes.
What is the difference between ptosis surgery and blepharoplasty?
Blepharoplasty removes excess skin, whereas ptosis surgery tightens the muscle tendon that lifts the lid. A low lid edge needs ptosis repair, a skin fold needs blepharoplasty, and many patients over 50 need both in one operation.
Is infrabrow excision better than upper blepharoplasty?
Neither is better, because they suit different lids. Infrabrow excision keeps the existing crease and suits thick-skinned outer hooding with a low brow. Blepharoplasty suits thinner skin and anyone who wants a crease created or raised.
How long does non-surgical eyelid tightening last?
Mono-polar RF (XERF, Thermage Eyes) and HIFU results last about 1-2 years, needle RF about a year, and a Botox brow lift 3-4 months. All need repeating, whereas surgery lasts a decade or more.
