What AGNES and Secret RF are
Both devices deliver radiofrequency (RF) heat through needles, but they act on different layers of the lower lid.
- AGNES uses a single or triple needle that is insulated along its shaft, so that only the exposed tip releases energy. The heat is delivered deep in the fat pad while the skin above is spared. Fat cells at the tip are destroyed, and the orbital septum, the sheet of tissue that holds the fat in place, contracts as it heals, which draws the bulge inward.
- Secret RF is a fractional microneedle RF device. An array of fine needles, typically 25 or 64, penetrates to a set depth of 0.5 to 3.5 mm and heats the dermis in a grid of small columns. The heat stimulates new collagen over the following weeks, which firms the crepey, lax skin of the lower lid. The needles do not reach the fat.
The “AGNES + Secret” protocol is a sequence. AGNES is applied to the fat pads first, and Secret RF to the lid skin, either in the same session or at a later one.
Who AGNES and Secret RF suit
- Mild-to-moderate fat-type eye bags, where the bulge is present at all times of day and more prominent on looking up (see the self-check), with some skin laxity.
- Patients who decline surgery or cannot take two weeks of downtime.
- Large fat bags are under-treated, fluid-type puffiness has no fat to shrink, and festoons (mounds of lax skin and retained fluid on the upper cheek, below the lid) lie beyond the reach of both devices.
What the evidence says about AGNES and Secret RF for eye bags
- AGNES-type needle RF for the fat, Grade C. One small clinical study measured a real reduction in the fat bulge that held for six months, with swelling and bruising for about a week. No study compares it with surgery or follows patients beyond six months.
- Secret RF for the skin, Grade C. One small study showed firmer, smoother periorbital skin. It did not measure eye bags.
- The combination, Grade D. No study has tested AGNES and Secret RF together for eye bags. “40% reduction” and “permanent” are clinic claims rather than measured outcomes.

A quoted package price should be checked for whether it covers both devices and how many passes of each, because the two are often priced separately.
AGNES and Secret RF sessions, downtime and recovery
- Procedure time. 30 to 45 minutes per side under topical and local anaesthetic.
- Downtime. 4 to 7 days of swelling and bruising after each AGNES session, and 1 to 3 days of redness after Secret RF. Bruising occurs in roughly one patient in ten.
- Result timing. Visible from 4 weeks and assessed at 3 months after the final session, because collagen remodelling and clearance of the treated fat continue over that period.
- Longevity. Clinics quote 1 to 3 years. The only published study stops at 6 months.
AGNES and Secret RF risks
Swelling, bruising, temporary numbness and small pinpoint marks are expected. Burns and pigment change are rare, and occur when the needle insulation fails or the settings are too high, because heat then reaches the skin the insulation was designed to spare. Under-correction and asymmetry need additional sessions. The needle passes close to the orbital rim, so the operator’s experience with the eye area matters more than the device brand.

Is AGNES + Secret RF permanent?
Fat cells destroyed by heat do not regenerate, but only part of the pad is treated and ageing continues. A top-up is usually needed after 1 to 3 years, and there is no published data beyond six months.
How many sessions of AGNES are needed for eye bags?
Two to three sessions, four weeks apart, is the usual protocol. The published study used two.
Does Secret RF remove eye bags on its own?
It does not. Secret RF tightens skin, so on a fat bag it changes the surface and leaves the bulge in place.
Is AGNES or surgery the better choice?
Surgery removes more fat in one procedure and has far more published outcome data. AGNES suits a mild bag or a patient who will not have surgery.
