Overview

Ultherapy, made by Merz Aesthetics, is a micro-focused ultrasound device with built-in imaging that places points of heat at set depths beneath the skin. Its role around the eyes is limited to the brow and the cheek below the eye socket, because it is cleared in the United States for lifting the eyebrow and its instructions advise against treating the eyes, the eyelids or the area within the orbital rim. After full-face and neck treatment, a rater-blinded study measured a brow lift of 1.7 mm at 90 days (Grade B), which softens upper-lid hooding indirectly. In Asian patients, however, the lift was 0.47 mm and had returned to baseline by six months. The only lower-lid study treated seven patients on the lid itself, an approach current instructions advise against (Grade C), and no study shows a reduction in eye-bag fat (Grade D). An eye-area session costs S$400 to 1,200 in Singapore in 2026.

Introduction

Ultherapy is the best-known focused-ultrasound brand in Singapore, and clinics offer it for hooded eyes, crepey under-eyes and eye bags, although its cleared use around the eye is limited to lifting the brow. This review covers what Ultherapy can and cannot treat around the eyes, the study results, the safety limits near the eye and 2026 prices.

What Ultherapy is and how it treats the eye area

Ultherapy converges ultrasound beams at a chosen depth, raising the temperature at each focal point to about 60 to 70°C while the skin surface is left intact. Separate transducers treat at depths of 1.5, 3.0 and 4.5 mm, and the built-in ultrasound image shows the operator the skin, fat, muscle layer and bone, so that the depth can be confirmed before each line is delivered. The heat contracts collagen at each point, and new collagen forms over the following 2 to 3 months. Ultherapy PRIME, the newer console, was cleared in the United States in February 2024.

  • Brow and forehead Lines of treatment above the brow lift it slightly and draw up the skin of the upper lid. Brow lifting is Ultherapy’s only cleared use in the eye area.
  • Under the eye Current instructions place treatment on the cheek below the bony rim of the eye socket, not on the lower lid.
  • The eyelids The eyelids are not treated, because the manufacturer’s instructions advise avoiding the eyes, the eyelids and the area within the orbital rim.

Ultherapy brow lift for hooded eyes

  • EvidenceEvidence for brow lift, Grade B. A mean brow lift of 1.7 mm at 90 days was measured in a rater-blinded study of 35 patients treated on the forehead, temples, cheeks, under the chin and on the neck.
  • Asian patients A study of 25 Asian patients given full-face and neck treatment measured a brow lift of 0.47 mm at 90 days, which had returned to baseline by six months.
  • Hooding, Grade C (indirect) A raised brow reduces hooding only indirectly, by drawing up the lid skin, and no study has measured Ultherapy’s effect on the hooded lid itself. The brow figures above come from full-face and neck treatment rather than from a brow-only session.

Ultherapy suits mild hooding caused partly by a low brow. Loose lid skin responds better to a treatment applied to the lid itself, such as Thermage Eyes, and skin resting on the lashes usually requires upper blepharoplasty.

Ultherapy under eyes and for eye bags

  • EvidenceEvidence for under-eye skin, Grade C. The only lower-lid study, published in 2014, treated seven patients on the lower lid itself to tighten the eye muscle and the septum, the sheet of tissue that holds the fat in place. Computed tomography scans measured a change of about 0.5 mm in the bulge of the septum, and no adverse events occurred. With a sample of seven, the finding is preliminary, and direct lid treatment is no longer the labelled approach because current instructions advise against treatment within the orbital rim.
  • EvidenceEvidence for eye bags, Grade D. No study has measured a reduction in eye-bag fat after Ultherapy, and treatment placed below the rim does not reach the fat pads, which lie within the orbital rim.

Ultherapy eyes before and after results timeline

  • Immediately Little visible change occurs, although the skin may be red and swollen for a few hours.
  • 2 to 3 months The measured brow lift appears as new collagen forms, and the 1.7 mm figure was recorded at 90 days.
  • 6 months In Asian patients the brow had returned to baseline by six months, although face and neck studies report improvement lasting up to one year.
  • 12 to 18 months A repeat session is usually given at this point.

Ultherapy is priced by lines, the rows of ultrasound pulses delivered, so two quotes can be compared only when both state the line count. Some prices exclude GST.

Ultherapy eye procedure, pain and downtime

  • Treatment The operator checks the ultrasound image, sets the depth and delivers each line of pulses, each of which produces a deep, prickling heat.
  • Pain Pain averaged about 5 out of 10 across 785 patients pooled in a 2025 meta-analysis, and numbing cream or oral pain relief is commonly used.
  • Downtime None to one day, although the skin can feel tender or numb for a few days.
  • Sessions One, repeated every 12 to 18 months.

Ultherapy eye side effects and safety

  • Common Redness, swelling, tenderness, welts and occasional bruising, which resolve within days.
  • Nerves Temporary numbness or weakness has been reported where the heat irritates a nerve.
  • The eye Case reports describe permanent corneal scarring, cataract, and iris and lens damage after focused-ultrasound treatment on or near the eyelids, mostly performed without eye shields. These injuries are rare, but the thin eyelid allows focused energy to reach the cornea and lens, and treatment therefore stays outside the orbital rim.

Ultherapy and Botox brow lift both raise the brow with Grade B evidence. Botox lasts 3 to 4 months at a lower price per session. Face and neck improvement after Ultherapy lasts up to about a year, although in Asian patients the brow returned to baseline by six months. Can Ultherapy be used under the eyes?


Ultherapy can be used below the bony rim of the eye socket, but not on the lower lid itself, because the manufacturer advises against treatment within the orbital rim. The only lower-lid study, from 2014, treated seven patients on the lid itself (Grade C).

Does Ultherapy remove eye bags?
