Clinic pages describing “4D eye bag removal” or “permanent 4D eye bag removal” describe a single-session procedure. A patented radiofrequency (RF) microneedle “selectively targets and shrinks subcutaneous fat” after numbing cream and local anaesthetic, the procedure takes about 20 minutes, and results are claimed to last “five years or more”, with one clinic citing “15 years of follow-up”. The technology described belongs to the same category as AGNES-type insulated-needle RF (see AGNES + Secret RF), delivered as one heavier session instead of a course. “4D” is a brand name rather than a technical standard.
What Fotona 4D and SmoothEye are
Fotona 4D is a four-step facial laser protocol that uses two wavelengths, a 2940 nm erbium laser and a 1064 nm neodymium laser. The four steps are a pass from inside the mouth, a non-ablative “SMOOTH” mode pass, a deep heating pass with the 1064 nm beam and a light ablative peel. SmoothEye is the periorbital application of the SMOOTH mode. The lid skin is heated in bulk without ablation, and the heat contracts existing collagen and stimulates new collagen, which tightens lax lower-lid skin. The treatment acts on skin and not on fat.

Risks of 4D eye bag removal and SmoothEye
The RF needle carries the same risks as AGNES, namely swelling, bruising, rare burns and under-correction. The laser carries transient redness and a rare risk of pigment change in darker skin types, and eye shields are required because the 1064 nm beam can injure the retina.
