Scarless eye bag removal is lower eyelid surgery through the inside of the lid: the fat pads are removed or moved into the tear trough with no external scar. It is the eye bag treatment with the most published outcome data (Grade B) and the only one that removes fat permanently in one procedure. In Singapore it costs S$5,000 to 10,000 all-in in 2026, needs 7 to 14 days of downtime, and suits moderate-to-severe fat bags with reasonable skin tone; loose skin needs a skin pinch added.

Scarless eye bag removal is lower blepharoplasty performed through the inside of the eyelid, so the fat pads are removed or repositioned without a skin incision. What scarless eye bag removal is

A 1 to 1.5 cm incision is made on the conjunctiva inside the lower eyelid, below the tarsal plate (the firm plate of tissue that gives the lid its shape). Through it the surgeon reaches the three orbital fat pads and either

  • removes the excess (fat excision), or
  • repositions it over the orbital rim into the tear trough (fat transposition), which fills the hollow beneath the bag at the same time.

The conjunctival incision is left to heal on its own or closed with a fine absorbable suture, and nothing is visible from outside. The procedure takes 45 to 90 minutes under local anaesthetic with sedation.

Who scarless eye bag removal suits

  • Moderate-to-severe fat-type eye bags with reasonable skin tone.
  • Younger patients with familial fat prolapse, where the skin does not need to be removed.
  • Fat repositioning suits a bag with a deep tear trough beneath it.
  • Marked skin laxity is not corrected by fat surgery alone. A skin pinch is added (see Pinch blepharoplasty), or a subciliary approach through an incision just below the lashes is chosen instead.

What the evidence says about scarless eye bag removal, Grade B

Scarless eye bag surgery has more published outcome data than any other eye bag treatment, including large patient series, systematic reviews and a 2026 series of over 500 Asian patients with high satisfaction. Because the incision is inside the lid, the complications that matter most, the lid turning out or pulling down, are least likely with this route. The common after-effect is temporary swelling of the eye’s surface (chemosis), which settles in about a month. No trial has compared surgery head-to-head with non-surgical options.

Scarless eye bag removal risks

Chemosis is common and settles on its own. Lid retraction and ectropion are the serious complications, and they are least likely with the transconjunctival route because the skin and the orbicularis muscle that support the lid are left uncut. Over-removal of fat produces a hollow that later needs filler, so repositioning has largely replaced removal. Rare complications are bleeding behind the eye, infection and double vision from injury to an eye muscle.
