Overview

Pinch blepharoplasty removes a narrow strip of loose lower-eyelid skin through a line hidden just under the lashes, leaving the muscle intact. It is the standard add-on when scarless fat surgery leaves crepey skin behind, and a stand-alone option for laxity-type eye bags. Evidence is Grade C alone and Grade B as part of lower eyelid surgery. In Singapore it adds S$1,000 to 2,000 to scarless surgery or costs S$3,000 to 6,000 on its own; sutures come out at 5 to 7 days and the fine scar fades over 3 to 6 months.

What pinch blepharoplasty is

With the patient looking up, the surgeon pinches the redundant lower-lid skin with forceps to judge how much is spare, marks it, and excises a strip of skin only, 2 to 4 mm wide, immediately below the lash line. The orbicularis muscle is left intact, which is what makes the pinch safer than a full subciliary blepharoplasty. The muscle continues to support the lid, so the risk of the lid pulling down (retraction or ectropion) is lower. The wound is closed with fine sutures, which are removed at 5 to 7 days.

It is performed

  • with transconjunctival fat removal or repositioning in the same sitting, the fat handled from inside and the skin from outside, or
  • alone, for laxity-type bags with little fat, or
  • with laser resurfacing of the remaining lid skin.

Who pinch blepharoplasty suits

  • Crepey, folding lower-lid skin (laxity-type bags, see the self-check).
  • Patients having scarless fat surgery whose skin will not retract on its own, typically those over 40, because the elastic recoil of the skin declines with age.
  • Fluid-type puffiness is not treated, and a large fat bag is not sufficiently corrected by the pinch alone.

What the evidence says about pinch blepharoplasty, Grade C alone and B as part of surgery

A 2026 series of patients who had a skin pinch with laser resurfacing showed improved lower-lid appearance with no change in lid position and no cases of the lid pulling down. As a component of lower eyelid surgery, skin excision is covered by large reviews that report low complication rates when the muscle is left intact.

Stand-alone skin-pinch prices are rarely published. Among the sources, one plastic-surgery clinic quotes incisional lower-eyelid surgery at S$5,000 to 8,000, and as an add-on to transconjunctival surgery (S$5,000 to 10,000) the pinch typically adds S$1,000 to 2,000. A stand-alone pinch can be expected to cost S$3,000 to 6,000, depending on surgeon type and facility.

Pinch blepharoplasty risks

The risks are a visible scar in the early months, small asymmetries, over-excision that pulls the lid down or exposes white below the iris (scleral show), and dry eye for a few weeks. Removing too much skin is the error to be avoided, so the excision is kept conservative and the muscle is not cut.
