Surgical and non-surgical eye bag removal both act on the orbital fat pads that form the bag, but surgery removes or moves the fat, whereas the non-surgical option shrinks only part of it. Scarless eye bag surgery (transconjunctival lower blepharoplasty) removes or repositions the fat through the inside of the lid in a single procedure. It has the most published outcome data (Grade B), costs S$5,000 to 10,000 in Singapore in 2026 and requires 7 to 14 days of downtime. Non-surgical eye bag removal with AGNES radiofrequency (RF) heats and partly shrinks the fat through an insulated needle. It rests on small studies (Grade C) and costs S$1,000 to 2,500 a session, or S$3,000 to 6,000 for a course of 2 to 3 sessions, with 4 to 7 days of swelling after each session. Surgery suits moderate to large fat bags, whereas AGNES suits mild to moderate bags in people who decline surgery. Which eye bag types suit surgical or non-surgical removal

  • Fat bags, moderate to large The bulge is present throughout the day and enlarges on upward gaze, because looking up pushes the orbital fat forward against the weakened septum. Surgery is the only option that reliably flattens it.
  • Fat bags, mild AGNES RF can shrink a small bulge, and surgery remains the alternative where a more complete result is wanted.
  • Fluid puffiness The swelling is worse in the morning and after salt or alcohol, because fluid collects in the loose lid tissue overnight while the head lies flat. Neither surgery nor RF improves it, whereas a raised sleeping position, lower salt intake and allergy control can.

How scarless eye bag surgery and AGNES RF treat eye bag fat

  • Scarless eye bag surgery The surgeon makes an incision in the conjunctiva, the lining on the inside of the lower lid, and either trims the bulging fat pads or moves them down into the tear trough to smooth the junction between lid and cheek. Because no skin is cut, no external scar forms. Where the skin is loose, a narrow strip can be removed beneath the lashes in the same procedure.   • Non-surgical eye bag removal (AGNES RF) A fine needle, insulated along its shaft, is inserted into the fat pad, and RF energy released at the uninsulated tip destroys fat cells and tightens the septum, the sheet of tissue that holds the fat in place. The insulation confines the heat to the tip, which protects the overlying skin from burns. Clinics often add Secret RF microneedling to tighten the skin. What studies show on non-surgical vs surgical eye bag removal

  • EvidenceEvidence for scarless surgery, Grade B. In a series of 69 patients who had fat repositioned through the inside of the lid, the tear-trough grade improved from 3.2 to 0.8 at 7 months, and no major complications occurred. A 2025 systematic review of 36 studies of lower-lid surgery reported a low complication rate and no major complications affecting vision. In an East Asian series of 439 patients, 10% of those who underwent fat repositioning were dissatisfied with the result.
  • EvidenceEvidence for non-surgical eye bag removal, Grade C. The evidence for needle RF on eye bag fat consists of small studies with follow-up of up to 6 months. In a related randomised split-face trial of 15 patients, three sessions of microneedle fractional RF, a different RF device, reduced the depth of the middle fat pad on ultrasound from 10.2 to 8.0 mm at 3 months, a reduction of about one fifth.

AGNES downtime recurs after each of the 2 to 3 sessions, so the swelling across a full course totals about 8 to 21 days, overlapping with the 7 to 14 days after surgery.

Risks of non-surgical and surgical eye bag removal

  • Surgery Bruising and swelling are expected for 1 to 2 weeks. The main lasting risks are lid retraction or ectropion (a lower lid pulled down or turned outwards), asymmetry, and excessive fat removal, which leaves a hollow. Lid malposition is less common with the inside approach, because the skin and muscle that support the lid are not cut. When skin is also removed, a canthal support stitch, which anchors the outer corner of the lid to the bone, lowers the risk.
  • AGNES RF Common effects are swelling, bruising, temporary numbness and pinpoint marks. Burns or pigment change can occur if the needle insulation fails or the settings are too high. Large bags are often under-corrected, because only part of the fat pad is treated, and may need surgery later.
  • For both, the operator’s experience with lower-lid procedures has a greater influence on the outcome than the device or the name of the technique.

Fat cells destroyed by AGNES RF do not regrow, but only part of the pad is treated, so the bag can return as the septum continues to weaken with age. Clinics advise a repeat course after 1 to 3 years, although published follow-up extends only to 6 months.

Which is better, AGNES or eye bag surgery?


Surgery removes more fat in one procedure and has more published outcome data (Grade B against Grade C). AGNES suits a mild bag, or a patient who declines surgery and accepts a partial result.
