What tear trough filler is and how it removes an under-eye shadow
The tear trough is the groove that runs downward and outward from the inner corner of the eye, where the lower lid meets the cheek. Overhead light casts a shadow in the groove, which is perceived as a dark circle. A soft HA gel placed on or just above the bone beneath the groove raises its floor level with the cheek, so that light falls evenly across the junction and the shadow disappears.
- The gel Products designed for this area are softer and bind less water than cheek fillers, which limits post-treatment swelling. One 1 ml syringe usually treats both eyes, and some patients require two.
- Technique A blunt cannula is standard, because its rounded tip is less likely than a sharp needle to pierce a blood vessel. In the 333-patient trial, needle and cannula produced the same response rate.
- Reversibility The enzyme hyaluronidase dissolves the HA gel if a lump, blue tint or vessel complication arises.
- Other names Clinics list the treatment as under-eye filler, tear trough filler or infraorbital hollow filler (infraorbital denotes the area below the eye socket).

A shadow from a hollow disappears under flat frontal light, whereas brown pigment remains unchanged when the skin is stretched. Tear trough filler results in randomised trials and meta-analyses
- EvidenceEvidence for under-eye hollows, Grade A. In a multicentre randomised trial of 333 patients, 87.4% of treated patients improved by at least one grade of hollow severity at 3 months, compared with 17.7% of untreated controls. The response persisted in 63.5% at 12 months and reached 80.3% at 18 months among patients retreated at 12 months. In a second randomised trial, which analysed 135 participants, most treated patients were satisfied and did not report being moderately or very bothered by dark circles at 3 and 12 months.
- Pooled results Across 31 reports and 2,556 participants pooled in a meta-analysis, 91% of patients were satisfied with the result.
- EvidenceEvidence for dark circles, Grade B. The trials measure the hollow directly but assess the darkness mainly through patient reports, and in the 333-patient trial at least 76% of patients reported reduced under-eye shadows. A 2026 study of 33 patients with moderate or severe hollows recorded fewer patients bothered by dark circles after treatment than before.
- Cannula technique in practice In a prospective study of 24 patients treated by cannula with no more than 1 ml for both eyes, every patient noticed improvement, 75% were satisfied, and the remaining 25% would have preferred more filler.
Tear trough filler before and after, from treatment day to 18 months
- Same day The hollow is filled immediately, and patients in the 333-patient trial reported recovery within 3 to 5 hours.
- Day 1 to 7 Swelling settles over 1 to 3 days, and bruising in a minority of patients lasts up to a week.
- 2 weeks A review visit takes place once the swelling has settled, and a small touch-up can be added if the hollow has been insufficiently filled.
- 12 months In the largest trial, 63.5% of treated patients were still responding.
- 9 to 18 months Clinics usually schedule a top-up within this window, and repeat treatments often require less product because some gel from the first treatment persists.
Before-and-after photographs on clinic websites show selected cases. The apparent depth of a tear trough also depends on lighting, because overhead light deepens the shadow and frontal light flattens it, so a fair comparison requires identical lighting in both photographs.

Tear trough filler procedure, pain and downtime
- Preparation Make-up is removed and a numbing cream is applied. Many fillers also contain the local anaesthetic lidocaine, which reduces discomfort during injection.
- Treatment The gel is placed deep in the groove through a cannula, and the procedure usually takes 20 to 30 minutes. Patients in the 24-patient cannula study rated the pain as very low.
- Downtime Swelling lasts 1 to 3 days, and a minority of patients bruise for up to a week.
- Sessions One treatment session, with a review at 2 weeks and repeat treatment every 9 to 18 months.
Tear trough filler side effects and risks
- Common In the meta-analysis of 2,556 patients, swelling occurred in 19%, bruising in 18%, redness in 7% and lumps or contour irregularity in 5%.
- Blue tint (Tyndall effect) A blue-grey tint appears when gel placed too close to the surface scatters blue light back through thin skin. It occurred in 0.9% of patients in the meta-analysis and resolves once the gel is dissolved with hyaluronidase.
- Delayed puffiness and lumps In a systematic review, delayed complications appeared on average 16.8 months after treatment, and most delayed swelling followed HA filler. Lumps were more often linked to semi-permanent and synthetic fillers, and permanent fillers are avoided in the tear trough because a lump from a permanent product cannot be dissolved.
- Vessel blockage In rare cases, filler enters and blocks an artery, which can cause skin loss or, very rarely, blindness. Blindness can result because the arteries around the eye connect to the ophthalmic artery that supplies the eye. The tear trough is regarded as a higher-risk filler site, and cannula technique, small volumes and hyaluronidase kept on site are therefore standard precautions.
