What the treatment evidence tells us

Choose treatment after identifying the cause

A Singapore clinical study identified several forms of periorbital darkness, including vascular, constitutional, post-inflammatory and shadow-related presentations. Its relevance here is the need to identify the mechanism; it does not compare clinics or establish a population-wide prevalence[27].

Filler evidence applies to suitable hollows

Randomised evidence supports HA filler for selected infraorbital hollows. This can improve the contour responsible for a shadow. The study population and outcome do not establish that filler clears brown pigmentation or suits prominent bags and fluid-prone lower lids[28].

Skin quality is a separate treatment target

Small clinical trials of polynucleotide injections and RF microneedling report improvements in some periocular skin parameters. The PN trial enrolled 27 people and the RF study 21 women. These findings do not establish reliable pigment clearance, equivalence to deeper volume correction or superiority of any clinic[31, 32].

Laser results depend on the protocol and target

A small prospective study reported improvement in pigmented circles after a combination of Q-switched Nd:YAG laser and RF-assisted vitamin C. Because it tested a combination without a laser-only control, it cannot establish that every laser or branded dark-circle programme gives the same result[33].

Discuss the risks of under-eye injections

HA filler can cause prolonged swelling, blue-grey discolouration or uneven contours. Rare vascular complications can cause tissue damage or visual harm. Discuss the injector’s relevant experience, patient selection, follow-up and response to a suspected complication before proceeding[29, 30].

Use the eye bag pathway when structure dominates

Blepharoplasty addresses excess skin and fat-related bulges through removal or redistribution of tissue. Improving those structures can reduce a related shadow; it does not establish treatment of brown pigment in the skin[34, 27].

Are dark circles and eye bags the same problem

Is laser or filler better for dark circles
